29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed were not de-energized before the employee works on or near them: Pier 3, Crane 2, Machine House: On March 10, 2017, a crane heavy equipment mechanic removed a rear metal panel cover of the Transformer Secondary Substation System (TSS) equipment near two energized 4160 volts metallic bus bars. The TSS was not de-energized prior to the work, the mechanic contacted the bus bars and was electrocuted.
