29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: The employer is failing to protect employees from struck-by and crushed-by hazards associated with plumbing and trenching activities. This was most recently documented on December, 20, 2017 at the jobsite located at or near 9750 East 53rd St N. in Bel Aire, Kansas. The excavation was not inspected by a competent person prior to the employees entering the trench and as needed throughout the shift.
