29 CFR 1910.269(l)(3)(iii): The employer did not ensure that no employee approaches or takes any conductive object closer to exposed energized parts than the employer's established minimum approach distance. Bronx, NY - Hellgate Substation 46: An electrician entered the MAD and was exposed to energized, high voltage equipment (12.5 kv or 25kv lines) while attempting to change fuses on a transformer on or about December 5th, 2019. NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
