29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use: a) Churn Room (Floor Level) - On or about December 10, 2019, product had dripped from the sample port on vessel P40 to the floor. b) Churn Room (Mezzanine Level) - On or about January 7, 2020, a process line between the vessel ROSS and vessel P20 was open and the valve was not car-sealed.
