29 CFR 1926.1204(a): The employer did not implement the measures necessary to prevent unauthorized entry: A) 133 East Clarke Place, Bronx - NY: The employer directed two elevator mechanics to perform work in a permit-required confined space where they were exposed to stored energy from an unsecured elevator car suspended above the elevator pit. The elevator access doors in the basement level were left open when the elevator mechanics were in the pit; thereby allowing unauthorized entrance on or about February 18, 2021. NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
