29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: a. 100 Middlesex Ave., Suite 2, Carteret, NJ - Warehouse The employer did not certify that employees assigned to operate the forklifts were evaluated to operate the equipment. Employees and staffing agency employees engaged in warehousing activities operated Toyota and Crown powered industrial trucks and were not provided an evaluation and complete a certification that included the name of the operator, training/evaluation dates, and identity of person performing the certification prior to being authorized to use the equipment. Violation occurred on or about April 12, 2021
