29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Location: a) Supported scaffold on ground level of the jobsite: On or about June 28, 2022, an employee was working on the fa�ade of the building while standing on the first tier platform of a two-tier-high supported scaffold that was not fully planked or decked between front upright and the guardrail supports, was not braced to prevent swaying or displacement. Employees did not have safe mean of access to the scaffold platform. Employee was exposed to falling up to approximately 6 feet to the lower level and was not given training to recognize fall hazards associated with the supported scaffold.
