29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).: On or about October 2, 2022 the Evergreen Greenhouse Services LLC at jobsite located at 7554 State Route 109 Delta OH 43515, failed to ensure employees were adequately protected from cave-in hazards while performing heat piping installation and repair activities in a 6 feet and 4 tenths or deeper trench.
