29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about December 12, 2014 and times prior thereto, employees of Mid-America Steel Products, Inc. manufactured trailer parts in the metal corrugated building with the use of powered industrial vehicles located at or near 909 C. North Missouri Avenue in Corning, Arkansas. The employer did not require or ensure employees operating powered industrial vehicles wore seatbelts that were equipped, exposing employees to the hazards of being thrown from and/or struck by vehicle.
