29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: At this oil waste treatment and disposal facility, employees were exposed to struck-by, caught-in, amputation and electrical hazards when performing service and/or maintenance. Energy control procedures with all required elements were not established and utilized by the employer for the following types of machines: a. Offload Bay 1, 2, 3, 4 and 5 (Pump Systems MCM); b. Charge Pump 1, 2 and 3 (Pump System MCM); c. Injection Pump 1 and 2 (Pump System Borretts); d. Oil transfer Pump (Pump System TITAN 3); e. Containment Sump (Pump System TITAN 4); f. SPF Assets (Pump System Gormann Rupp); g. Main Transfer pump (Pump System RUFF); h. Secondary Transfer Pump (Pump System Vaughn); i. Triplex Transfer Pump (Pump System TSC), and; j. Vacuum Unload System (Vaughn).
