29 CFR 1926.452(i)(4): Outrigger beams were not secured in place against movement, and were not braced at the fulcrum point against tipping. a. At the jobsite located at 203 Bay Dr. N Bradenton Beach, Florida, East side of the building - On or about June 23, 2021, the supported scaffold outrigger beams were not braced, exposing employees, installing the fascia, to a 25 foot fall hazard.
