29 CFR 1910.119(e)(1): The employer did not perform an initial process hazard analysis (hazard evaluation) on processes covered by 29 CFR 1910.119: a) MFG Chemicals - Callahan Location: The employer did not conduct an initial Process Hazard Analysis (PHA) for all processes covered under process safety management. The employer did not establish a process evaluation to address industry recognized hazards, previous incidents, engineering and administrative controls, consequences for failure to documenting administrative controls and engineering controls, facility siting, human factors and evaluation of safety and health of the employees workplace. Employees were exposed to fire/explosion hazards, chemical run away reactions, and other hazards while performing assigned duties for the manufacture of industrial chemicals.
