29 CFR 1910.146(d)(4): Provide the following equipment (specified in paragraphs (d)(4)(i) through (d)(4)(ix) of this section) at no cost to employees, maintain that equipment properly, and ensure that employees use that equipment properly: On or about August 28, 2014, employees entering permit required confined spaces were not provided with the required equipment to include but not limited to testing and monitoring equipment, communications equipment, emergency rescue equipment and/or a ladder exposing employees to atmospheric hazards.
