29 CFR 1926.452(i)(5): The inboard ends of outrigger beams were not securely anchored either by means of braced struts bearing against sills in contact with the overhead beams or ceiling, or by means of tension members secured to the floor joists underfoot, or by both. a. At the jobsite located at 203 Bay Dr. N Bradenton Beach, Florida, East side of the building - On or about June 23, 2021, the supported scaffold inboard ends of the outrigger beams were not anchored, exposing employees, installing the fascia, to a 25 foot fall hazard.
