29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: a) Throughout facility: A Raymond vertical upright forklift model EASi-OPC30TT serial number EASi-98-AB20654 was not taken out of service when it was found to have a broken light that was needed for illumination when picking orders on the shelving racks, on or about 8/12/15. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
