29 CFR 1910.27(d)(3): The side rails and rungs of side step ladders did not continue a minimum of 3 1/2 feet above the landing platform: At the facility batch plants, employees accessed upper landing areas and the side rails of the ladders did not extend above the landing platform exposing employees to fall hazards up to approximately 45 feet to the ground below.
