29 CFR 1910.146(g)(2): Training was not provided to each affected employee entering a permit-required confined space: (a) 157 West Jackson Street, New Holland PA, Interior of Milk Receiving Area, Silo 50 - On or about June 22, 2022, Silo 50 was identified as a permit required confined space on the FDLE Confined Space Program; however, the employer did not provide permit required confined space training for employees entering Silo 50 to perform maintenance work. Employees were exposed to hazards to include caught in and engulfment hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
