29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources: a) Assembly Line/Clean Room - On or about 3/9/2019 and at times prior thereto, the employer exposed employees to caught-in hazards, in that employees replacing the clamp pad on GA 00-100 TM Case Turn Over # 1, utilized the E-Stop button instead of the Turn Over # 1 Case Over Tooling head support or a block (an energy isolating device).
