29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): JOB SITE: 108 Business Highway 61, Bowling Green, MO 63334, on or about 1/28/2020, employees were permitted to use a yellow flexible cord without visually inspecting the cord for defects. The flexible cord was used to power portable hand tools with a missing ground pin. The workers were exposed to an electrical hazard.
