29 CFR 1928.57(c)(5)(i)(A): Application of electrical power from a location not under the immediate and exclusive control of the employee or employees maintaining or servicing equipment was not prevented by providing an exclusive, positive locking means on the main switch which can be operated only by the employee or employees performing the maintenance or servicing: On or about March 10, 2020, at the gin building, a positive locking means (lockout device) was not applied at the electrical power source by each employee involved in maintenance work on the trash conveyor below the east cyclone bank. This potentially exposes employees to unexpected start-up of the trash conveyor motor and machine caught-by hazards.
