29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: a) On or about 03/05/14, in the establishment, employer did not outline the requirements for testing a vertical band saw located in the machine shop to determine and verify the effectiveness of lockout/tagout devices for employees performing maintenance/servicing activities such as, but not limited to changing saw's blades.
