29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. Yard (Tug 120862 VIN:4LMBB2113AL021588): On or about January 24, 2017, employees were exposed to struck-by hazards, in that the operators were allowed to move loads of up to 80,000 lbs. prior to receiving operator training on the use, care and inspection of the tug vehicles. The operators were observed using the tug vehicles without attaching the service brake air lines to the trailers thereby rendering the trailer brakes inoperative. b.. Yard (Tug 140348, SN 303614): On or about January 24, 2017, employees were exposed to struck-by hazards, in that the operators were allowed to move loads of up to 80,000 lbs. prior to receiving operator training on the use, care and inspection of the tug vehicles. The operators were observed using the tug vehicles without attaching the service brake air lines to the trailers thereby rendering the trailer brakes inoperative.
