29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects and for evidence of possible internal damage. On or about 19 July 2017, and at times prior thereto, an employee was using a 120-V extension cord where the outer sheathing was cut through to the internal insulated wires, exposing the employee to an electrical shock hazard.
