29 CFR 1910.119(j)(4)(i): Inspections and tests were not performed on process equipment to maintain its mechanical integrity: This condition was observed in the Aniline and DHN units of the DuPont facility in Beaumont, Texas, when the employer failed to inspect critical process equipment to identify deficiencies and have them corrected to assure the integrity of the process is maintained. Examples of deficiencies observed by the CSHO include: 1.Unsecured and/or unanchored piping supports, such as the support on the 3" cooling water crossoverline on the upper level of the Aniline Unit, which was found not securely anchored to the platform. 2.Damaged or inadequate piping supports, such as the pipe support o the 1" hot decanter vent line (J284) in the DHN, which was observed missing the clamping fastener/bolt and instead utilizing wire to keep it on the pipe. 3.Missing and/or damaged insulation on piping, such as the steamline on the upper level of the Aniline Unit that supplies steam for heat tracing, which was found not to be insulated and exposing operators to possible burns when they are conducting rounds through the area. 4.Missing piping components, such as the valve (P92C) on top of the North (#1) DHN Reactor, and the valve (SV577) on glass lined pipe 7110-1-7 of the DHN Unit. Both valves could not be operated because they were missing their handles. 5.Damaged piping components, such as external surface pitting observed on the body of the vacuum jet system at the purge column of the Aniline Unit, or broken steam trace lines. 6.Degraded coatings, such as missing, cracked or spalled paint on piping and vessels and equipment identification markers. 7.Severe and unarrested external corrosion on process piping and vessels, such as the bottom nozzle of the Letdown Separator (7020-06) and pipe spool A625 in the Aniline Unit. 8.Leaks in the heat trace system on several levels of the DHN Unit, including several areas where steam and condensate were spraying heavily into walkways that operators uses for access to equipment.
