29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: A. Loading Dock Area, Waste Grinder: On, or about, July 14, 2023, the outer insulation provided on a red extension cord was damaged, exposing the inner conductors. The extension cord was used to energize the Skilsaw reciprocating saw, Model SPT44. B. Assembly, Specialty Finish Area: On, or about, July 20, 2023, the outer insulation provided on the power cord for the Milwaukee polisher Cat. No. 5460 was split/missing.
