29 CFR 1910.263(c)(3): Sprocket(s) and V-belt drive(s) of bakery machinery located within reach from platform(s) or passageway(s) or located within 8 feet 6 inches from the floor were not completely enclosed: a) Baker Boys LLC DBA Formica Bros. Bakery - conveyor table in front of the bakery Dough Sheeter machine/ bread line area: The takewaway conveyor table # 14111 had an unguarded chain and sprocket located approximately 1.5 feet from the floor. The unguarded chain and sprocket caused an amputation of an employee's right forearm, occurred on or about 05/04/2016. b) Baker Boys LLC DBA Formica Bros. Bakery - Konig Dough divider machine /bread line area: The Konig dough divider machine also registered as Rex Automat Type T-4 S-MKB had an unguarded chain and sprocket located approximately 3.45 feet from the floor, the unguarded chain and sprocket exposed machine operators to amputations, occurred on or about 05/04/2016. c) Baker Boys LLC DBA Formica Bros. Bakery - Packing area: The Le-matic modular roll slicing machine model # RS-S, Serial #919188 had two unguarded chains and sprockets and one unguarded belt and pulleys located approximately 2.5 feet from the floor, the unguarded chains, sprockets, belt and pulleys exposed machine operators to amputations, occurred on or about 05/04/2016. d) Baker Boys LLC DBA Formica Bros. Bakery - Winkler machine/bread line area: The Winkler machine also known as hamburger bun maker had approximately 8 guards missing that covered the chains and sprockets located within 1 to 8 feet from the floor, the unguarded chains and sprockets exposed machine operators to amputations, occurred on or about 05/04/2016. e) Baker Boys LLC DBA Formica Bros. Bakery Paking area: The Doboy Machine (also known as single bagger) Model #SUD-ER, S/N 75-9586 (280Volts) had a guard missing to cover the chains and sprocket located within 3 feet from the floor, the unguarded chains and sprockets exposed machine operators to amputations, occurred on or about 05/04/2016.
