29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about August 28, 2014, the employer did not develop written lockout/tagout procedures for each type of equipment or machine located within the plant, to include but not limited to Punch press, Lathes, a De-coiler, Bridgeport Press Brake, and a Cincinnati Sheer. Employees performing maintenance, setup or servicing work were exposed to electrocution and/or amputation hazards.
