29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. On or about March 30, 2014, in the Heat Treat Facility, a Heat Treat Operator/Supervisor was tightening a 1 5/16 inch nut on the elevator cylinder inside the oil filled quench tank chamber of a AFE-6 Casemaster Atmospheric Heat Treat Furnace #21 and did not affix an energy isolating device to the pneumatic energy source for isolating the energy source to the outer metal door of the quench tank.
