29 CFR 1910.147(d)(5)(i): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. On or about March 30, 2014, in the Heat Treat Facility, a Heat Treat Operator/Supervisor was tightening a 1 5/16 inch nut on an elevator cylinder inside the oil filled quench tank chamber of the AFE-6 Casemaster Atmospheric Heat Treat Furnace #21 and did not properly restrain the pneumatic energy source for the outer metal door to the quench tank to prevent the door from coming down from its raised position.
