29 CFR 1910.179(j)(3): Periodic inspection. Complete inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated below. These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the following items. Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: a. 4400 S. Packers - North Receiving Bay - On or about September 23, 2014, the employer had not performed a periodic inspection on the Abell-Howe, Model TR, SG, DM, Serial # 83-17963-1, 10 Ton Crane, thereby exposing employees to fractures and crushing injuries. b. 4400 S. Packers - Production Bay - On or about September 23, 2014, the employer had not performed a periodic inspection on the Abell-Howe, Model # DG, UR, CD, Serial # ML-6918330-1, 5 Ton Crane, thereby exposing employees to fractures and crushing injuries. c. 4400 S. Packers - On or about September 23, 2014, the employer had not performed a periodic inspection on the Contryx, Model #GC2-16, Serial # 21091, 2 Ton - A Gantry Crane, thereby exposing employees to fractures and crushing injuries. d. 4400 S. Packers - On or about September 23, 2014, the employer had not performed a periodic inspection on the Contryx, Model GC2-16, Serial # 22752, 2 Ton - B Gantry Crane, thereby exposing employees to fractures and crushing injuries. e. 4400 S. Packers - On or about September 23, 2014, the employer had not performed a periodic inspection on the Contryx, Model GC2-16, Serial # 38929, 2 Ton -C Gantry Crane, thereby exposing employees to fractures and crushing injuries. f. 4400 S. Packers - On or about September 23, 2014, the employer had not performed a periodic inspection on the Contryx, Model GC2-16, Serial # 31444, 2 Ton - D Gantry Crane, thereby exposing employees to fractures and crushing injuries. g. 4400 S. Packers - On or about September 23, 2014, the employer had not performed a periodic inspection on the Unknown, E Gantry Crane, thereby exposing employees to fractures and crushing injuries. h. 4400 S. Packers - On or about September 23, 2014, the employer had not performed a periodic inspection on the Contryx, Model GC22-16NS, Serial # 22752, 2 Ton F Gantry Crane, thereby exposing employees to fractures and crushing injuries. i. 4400 S. Packers - On or about September 23, 2014, the employer had not performed a periodic inspection on the Contryx, Model GC2-12, Serial # 31444, 2 Ton G Gantry Crane, thereby exposing employees to fractures and crushing injuries. j. 4400 S. Packers - On or about September 23, 2014, the employer had not performed a periodic inspection on the Contryx, Model GC2-16, Serial # 21089, 2 Ton - H Gantry Crane, thereby exposing employees to fractures and crushing injuries. k. 4400 S. Packers - On or about September 23, 2014, the employer had not performed a periodic inspection on the Contryx, Model GC2-16, 2 Ton - J Gantry Crane, thereby exposing employees to fractures and crushing injuries. l. 4400 S.
