29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: a) Rockvale 2 Laurel Site: On or about October 29, 2018 and at times prior, employees were exposed to potentially flammable and/or oxygen deficient atmospheres, and struck-by hazards while entering the Astec Double Barrel Drum Mixer to conduct grinding, cleaning, and basic maintenance operations. The employer failed to provide adequate training in the recognition of permit required confined space hazards and the methods to control these hazards to the employees required to enter a permit required confined space.
