29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry: a) Rockvale 2 Laurel Site: On or about October 29, 2018 and at times prior, employees were exposed to potentially flammable and/or oxygen deficient atmospheres, and struck-by hazards while entering the Astec Double Barrel Drum Mixer to conduct grinding, cleaning, and basic maintenance operations. The employer did not implement procedures that would ensure rescue and emergency services were available as needed for entrants to permit-required confined spaces.
