29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a) Notching Department - On or about August 26, 2016, the junction box used to power up the Notching machine was damaged allowing wood dust to accumulate inside the electrical box. (b) Cutting Department - On or about August 26, 2016, the junction box used to power up the Rockwell 10" Unisaw was damaged allowing wood dust to accumulate inside the electrical box. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.
