29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard. a) On or about 2/7/2017, Outside at the flume rack: Employees accessing the mobile grapple were not restrained to prevents falls into the water intake racks. b) On or about 2/15/2017, Outside at generator rack: Employees working on metal decking above the water intake grates were not restrained to prevent falls into the water intake racks. Abatement documentation is required for these items.
