29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Production area handmounting station: A 4x4 receptacle that was meant to be mounted to a surface was made into an extension cord and used as pendant box to power a handmounting machine, on or about 7/13/15. b) Production area next to Thomson Die Cutting machines: Two 4x4 receptacles were used as pendant boxes to power the die cutting machines, a phone charger and fans and were not mounted on a surface, on or about 7/13/15. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
