29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Concrete Mixing Drum: Employees did not use lockout devices on the cement mixer truck while chipping concrete from the interior walls while inside the mixing drum. On 03/01/2017 an employee was crushed to death by the rotating drum.
