29 CFR 1910.165(b)(1): The employee alarm system did not provide warning for necessary emergency action as called for in the emergency action plan, or for reaction time for safe escape of employees from the workplace or the immediate work area, or both: a) MFG Chemicals- Callahan: The employer did not ensure that a warning was sounded for employees, as called for in the employer's emergency action plan. On May 21, 2012, a run-away reaction occurred as a result of an increased temperature in the # D-6 reactor. The reaction resulted in over pressurization of the vessel, causing the rupture disc to blow. Employees were exposed to the hazards associated with hydroxgen peroxide, maleic acid and other materials. No warning sound was activated to notify employees to evacuate.
