29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about March 23, 2018, and at times prior thereto, the employees installed shop made fork extension attachments. They use the extensions when they are transporting the low profile tanks, which measure approximately 15 feet wide and 16 feet high. The forklift is a Hyster, H360XL. Employees are exposed to hazards such as, but not limited to being crushed by the load or the forklift tipping over.
