Bakery - Packing area: The on/off control junction box and the side control box on the Oliver Machine (also know as bread cutter) Model 797G, S/N 714820 had missing knockouts, observed on or about 05/04/2016. b) Baker Boys LLC DBA Formica Bros. Bakery - Packing area: A drop down outlet located near the Oliver machine had a missing knockout, observed on or about 05/04/2016. c) Baker Boys LLC DBA Formica Bros. Bakery - Packing area: A control button on the Doboy Machine (also known as single bagger) Model #SUD-ER, S/N 75-9586 (280Volts) had a missing knockout. Observed on or about 05/04/2016. d) Baker Boys LLC DBA Formica Bros. Bakery - in front of the baking area: A control button was missing on the Atwood Scale S/N 4696, Observed on or about 05/04/2016. e) Baker Boys LLC DBA Formica Bros.
