29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made. (a) CWC 226, Artillery Repair: Employees were exposed to electrical shock where the employer did not replace a damaged pendant cord. (b) CWC 271, HMMWV: Employees were exposed to electrical shock where the employer did not replace a damaged extension cord. (c) CWC 113, Material Expeditors bldg. 615: Employees were exposed to electrical shock where the employer did not replace a damaged extension cord. (d) CWC 227, M917 Dump Trucks: Employees were exposed to electrical shock where the employer did not replace a damaged extension cord. (e) CWC 123, Maintenance bldg. S634A: Employees were exposed to electrical shock where the employer did not replace a damaged extension cord. (f) In the cable shop employees were exposed to electrical shock from the use of damaged cords as flexible cords and extension cords were not visually inspected before use
