29 CFR 1910.305(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: a) Boiler Room: Employees were exposed to an electrical shock while performing maintenance, boiler treatment and other activities. The Square D panel box (120/240 volts) did not have a panel enclosure to protect the electrical circuits.
