29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about March 22, 2018, in the assembly department at the cutting and press area, the Bantam iron worker used to cut metal channel for the stairs and to punch holes, was not guarded. Employees were exposed to the hazards of being struck by the unguarded shear and punch operations. b) On or about March 22, 2018, in the assembly building, the rotating flywheel on the Bantam iron worker was not guarded. Employees use the iron worker to cut steel for stairs. Employees were exposed to the hazard of being struck by the rotating flywheel. c) On or about March 22, 2018, in the assembly building, the rotating belts on the Bantam iron worker were not guarded. Employees use the iron worker to cut steel for stairs. Employees were exposed to the hazard of being caught-in the rotating belts. d) On or about March 22, 2018, in the assembly building, the rotating pulleys on the Bantam iron worker were not guarded. Employees use the iron worker to cut steel for stairs. Employees were exposed to the hazard of being caught-in the rotating pulleys.
