29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) On or about February 6, 2018, in the fabrication shop, employees were working around an area where there was an energized 240 volt conductor lying on the floor with the outer sheath pulled back and the copper end unprotected, exposing employees to the hazard of an electrical shock. b) On or about March 29, 2018, in the fabrication shop, on the 110 volt 5 ton winch used to lift tanks lids, the electrical control panel covers were missing , leaving unprotected conductors open to contact, exposing employees to the hazards of electrical shock.
