29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about March 22, 2018, in the spray paint area, a 110 volt extension cord which was not equipped with a grounding pin was used to supply power to a heater, exposing employees to an electrical shock hazard.
