29 CFR 1910.261(k)(13)(ii): Where pulpers are located directly below the broke hole on a paper machine and where the broke hole opening is large enough to permit a worker to fall through, employee pushing broke down the hole were not wearing a safety belt attached to a safety belt line: a) On or about 2/1/2017, Machine room: The pulper located below the calender stack had openings of ten feet by 15 inches and ten feet by 21 inches. No safety belts were in use.
