29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Throughout facility: On or about 4/8/15 the employer did not record the following workplace injurie(s) or illness(es) on the OSHA Form 300 or equivalent for the calendar year 2015: a) Production area UV Coating machine: On 4/8/15 an employee cleaning the UV Coating machine with UV cleaner developed a rash on his arms. The employee was prescribed antibiotics. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
