29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Throughout facility: On or about 8/12/15 the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2015: a) Production area 12 foot weaving machine: On 7/30/15 and employee was attempting to fix the fingers on the turret of a 12 foot weaver machine and his two fingers were caught into a chain and sprocket and amputated. The employee was placed out of work due to the injury. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
