29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure: 12 Queen St (also known as 68 Jaques Ave), 72 Jaques Ave, and 26 Queen St: On or about 1/13/16, hepatitis B vaccine and vaccination series had not been made available to employees exposed to bloodborne pathogen hazards from contaminated sharps while performing housekeeping duties such as, but not limited to, emptying trash receptacles. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1). Crothall Healthcare was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.1030(f)(1)(i), which was contained in OSHA inspection number 1104003, citation number 1, item number 1(b), and was affirmed as a final order on 1/12/16, with respect to a workplace located at Guthrie Clinic, Hanshaw Rd, Ithaca NY.
