29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Facility Wide: Employer failed to report an in-patient hospitalization of a burn related injury occurring within 24 hours of a work related incident. On February 11, 2017, around 6:50 pm, an employee carrying corrosive material spill on his pants while performing his duties at Table Mountain Casino. The incident was reported to OSHA on February 15, 2107, at 2:50 pm.
