29 CFR 1904.29(a): A Log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injuries (OSHA Form 300A), and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment: a) Lumber /Mill Yard(Facility): On or about August 01, 2017, and at times prior, the employer failed to maintain the OSHA 300 Logs such as but not limited to Work-Related Injuries and Illnesses (OSHA Form 300) nor did a company executive certify that he or she examined the OSHA 300 Logs and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete.
